| Tag Replacement Form In the event the ID tag on your sheep that is listed on the registration papers is lost, when you replace the tag you can note new tag information on this form and attach this information to your registration papers. PRINT AND RETURN paper form or use the ONLINE form below and the new tag ID number can be corrected on your registration papers to match the new tag on your sheep. There is no charge when tag number correction is done at the same time as other paid work on the Certificate, such as transferring ownership. If you want the new tag listed on your papers without other work being done the charge is $6.50 to reprint a new certificate. You will need to mail the original certificate to: United Horned Hair Sheep Association, Inc. PO Box 161 New Lebanon, OH 45345 This form can also be used to add a Tattoo number or Microchip information to the sheep's information. Provide the new information below.
